JOB TITLE: REHABILITATION CO-ORDINATOR

(Realt Nua).

MAIN DUTIES AND RESPONSIBILITIES

OPERATIONAL MANAGEMENT

* Overall responsibility for the day-to-day operational management
of Realt Nua.
* To ensure all case and administrative records and files are
maintained to a high standard.
* Initiating and formulating policies, reports and plans for
adoption by the Board.
* Ensuring adequate health and safety policies and procedures are
devised, implemented, and reviewed.
* Ensuring appropriate risk management policies and procedures are
implemented and reviewed.
* Ensuring, with the management team that annual targets, budgets
and key performance indicators are met.

SERVICE DELIVERY

* To ensure quality and best standards are adhered to in all aspects
of the work.
* Overseeing the implementation of the Ballyfermot STAR CLG
strategic plan with the management team and supporting the
organisation to implement a broad range of programmes.
* Identifying gaps and securing new programme opportunities.
* Ensuring with the management team that the plans and objectives of
Ballyfermot STAR CLG build on existing work and are continuously under
review and evaluation.
* To support new initiatives and programme development by ensuring
that the staff team remain current in clinical trends and
developments.
* Carry out initial and comprehensive assessments of client need in
line with HSE National Drugs Rehabilitation Framework.
* To keywork and case manage people who are using substances by
supporting them to develop and implement their individual care plans.
* Ensure ongoing monitoring and review of care plan objectives.
* Make appropriate referrals to all internal and external services.
* Facilitate referrals and assessments using care and case
management programmes based on the HSE National Drugs Rehabilitation
Framework.
* To design and deliver psychoeducational workshops.
* To submit funding applications for resources where appropriate.

HUMAN RESOURCES MANAGEMENT

* To lead and manage the Realt Nua staff team according to best
practice.
* To promote a culture of best practice, professionalism and
compliance among the staff.
* To implement appropriate HR policies.
* To provide regular supervision to staff.
* To review the performance of team members on an annual basis.
* To ensure the performance management of the team is reviewed on an
annual basis.
* Managing Relationships
* To build strong relationships with key stakeholders including the
HSE, Ballyfermot Drugs & Alcohol Task Force, DSP, CDETB DCC, Pobal,
Public Representatives, and other community and voluntary services.
* To ensure that Ballyfermot STAR CLG, its mission, programmes and
services are presented consistently.
* Develop and execute an annual external communication plan that
positions Ballyfermot STAR as a rehabilitation leader in the sector
including high quality organisational publications.
* Ensuring good communications with the Realt Nua team members and
other teams withing the organisation.

IT AND COMMUNICATIONS

* Develop and oversee Ballyfermot STAR’s Realt Nua IT strategy and
infrastructure and ensure necessary systems that support and capture
the organisations activity are in place.
* Experience in social media management.
* Proficient in the use of Microsoft Office packages including
Microsoft Teams.

QUALIFICATION, KNOWLEDGE AND SKILLS REQUIRED

KNOWLEDGE AND SKILLS REQUIRED:

* Minimum of a level 7 Diploma in Community Drugs Work, Addiction
Studies, Community Development, Social Care or related field.
* Minimum of 5 years paid experience in
addiction/rehabilitation/community work/community development
settings.
* A minimum of 3 years paid managerial experience with a proven
track record in team leadership.
* Training in evidenced based models such as CRA, MI, CBT, SAOR.
* Experience in holding a client case load and a proven track record
in key working, care planning and case management.
* A knowledge and understanding of service planning, performance
monitoring and Quality Assurance.
* Have a thorough knowledge of substance related issues and their
effect and influence on individuals, families and communities.
* Knowledge and understanding of theory in evidence-based practice
in the field of treatment & rehabilitation.
* Knowledge and understanding of National Drugs policy including
Reducing Harm, Supporting Recovery: A health led response to drug &
alcohol use in Ireland * – **Apply on the

* Knowledge and understanding of National Drugs Rehabilitation
Framework and National Protocols and Assessment Guidelines.
* Good knowledge and experience of case management and interagency
collaboration.
* Have experience of high-quality inter-agency work, involving
community, voluntary and statutory agencies.
* Experience in facilitating training, development and workshops
with various groups.
* Effective written, oral and interpersonal communication skills,
including IT skills to ensure that all records, policies and
procedures are kept in line with GDPR and good practice guidelines.
* Excellent knowledge of boundaries and confidentiality.
* Experience in client data management systems such as eCASS & HRB.
* He/she must have administration capacity to discharge the
functions of the post.
